Whether you just have questions, need help setting up an account, comparing plans and/or filling out an application help is here. This service is free and open to the public. Glendale Area Medical Association, Inc., offers an in-house navigator to assist with your needs.

Glendale Area Medical Association, Inc., is dedicated to providing you with the highest quality, cost effective medical care and dental care. We accept all major credit cards and offer a sliding fee for those who qualify.

Additional Fees:
There is an additional $10 dollar fee for completion of forms for use outside of the patient's medical record, including:
