PRESCRIPTION POLICY

        The staff and medical providers at the Glendale Area Medical Center are committed to quality, accessible healthcare.  We are asking our patients to help us achieve this by using our phone answering system for prescription refills.

        When calling for a prescription refill, please be prepared with the following information:

• Name

• Birth Date

• Phone Number

• Name of Medication

• Milligrams (Mg) / Strength

• How many and how often it is taken

• Name of pharmacy

        Please also let us know if you need more than a one-month supply for insurance purposes.

        Prescriptions will be phoned to the pharmacy after 5:00 p.m., so the medication will not be available to you until evening.  Refill requests placed after 4:00 p.m. will be handled by the end of the next business day.  We also ask that you bring a complete medication list and the refills you need to every office visit.  We appreciate your cooperation in helping us to better serve the community.
